We came here 10 years ago on a visitor visa and haven't left since then. The problem is we don't have the I-94 form anymore which is where my question comes from. Under "Status at Entry" I put B2 Visitor Visa, but I'm really not sure about what to put for question 16. I feel like saying "No" would be incorrect because we DID have an I-94 at one point, but saying "Yes" requires me to enter the I-94 number in addition to the date my authorized stay expires. I have the old passport with the entry stamp on it which is one of the pieces of evidence I'm submitting along with everything else.
This was my same problem. I had an I-94, but I lost it. I know the entry date and the date it expired because my sister and I came on the same day, same flight.

What I did was, I put Yes for question 16a. Then on page 6, on the additional information section, I put an explanation saying that I couldn't find my I-94 but that I knew the date it expired because my sister and I came on the same day and she does have her I-94.

I got arrested for DUI in September but the judge gave me PBJ (Probation Before Judgement) for it. This basically means that I am not officially convicted of the DUI. I will not be convicted of it until I get arrested and convicted for another felony. So in all intents and proposes I am in the clear. But because I got arrested for it even tough I am not convicted, should I still check yes on it?

DUIs will be a cause for denial. You were arrested for it, so you were drving drunk right? I suggest you contact a lawyer.

If you can't afford an attorney look for nonprofit legal aid foundation that can look at your paper work more closely.

Ii have a few questions. So since i'm applying for consideration of deferred action for childhood arrivals. ((c)(33)) i have to send the i-765, the i-821D, I-765 WS, and G-1145 all to the same address ( P.O. BOX 805887 CHICAGO, IL 60680-4120)? And i have to send the total of $465? and all the documentation they want? all to the same address right?? if anyone can help me with this it would be great! Thank you anyone that helps me out with this situation.

Depends on where you live but for the Chicago Lock Box is P.O. Box 5757 as you can see from the link above.
