From past experience with RFE's I know that the sheet that you must include is the sheet that came with the RFE that says in caps that you must submit it along with your evidence. Noted, I just wanted to know if i can send in another envelope, and from what I can see. You did.
So I shouldn't worry.
Yeah, the RFE Letter says it should be on top of all the Requested Evidence.

I sent one for the year of '08
One for the year of '10
One for the year of '11
One for the year of '12 (June 15, 2012)
So I was basically missing '07&'09
I sent some Doctor Appointments from '07 Though
I don't recall sending anything from '09 though, that must have been the case.
I will just send them a bunch of evidence this time around.
Initially I did not want to overwhelm them with Information, but I see how far that decision got me.
