Hi guys, I have a question on 821D for travel information. If I have never left the US, do I still need to provide my passport information? It asks which country issued my passport?

I don't the answer to that question. I can only tell you that I put my passport info the first time i renewed and i plan on doing the same thing today.

I put it on my initial application but on both my renewals I put Lost (because I couldn't find it) and didn't have any problems.

is it required to take a picture of the back of the EAD card?

I just can't believe i misplaced my EAD card, and i have a feeling I'll find it when i least expect it.

I only took a picture of the front of my card for when the HR asked me to. Would that still be fine?

Finally Approved! So stocked! I was ready to take a leave of absence at work. My EAD expires on Nov18, so i was starting to panic. I made a service request call on the 7th, i was approved on the 8th. Not sure if it was because of that or just coincidence, but finally approved.
Btw, i'm not a religious person per say, but my dad introduced me to "San Expedito", or "St. Expeditus", i guess he just became my new saint. Anyways, stay strong fellas!
