NM DL revoked?

*Sorry this is in the wrong section...*
So I made an appointment with the MVD...and went to it, and passed all the tests, and even got my printed temporary license. Then, a few weeks later, today, I got a letter from the Tax and Revenue Dept. stating quite vaguely that the info was missing, incomplete, or improperly documented. I have to reapply for the DL now. Is this because I didn't use my Tax ID?

Has this happened to anyone else???
