Dental and eye exams.

Stupid question here does dental and eye exams counts as a medical record? I have a bunch of invoices and list of all the procedure done to me. How about ER invoices??

The key with this process is to "over-kill"
Meaning gather every single documents/ paper work you got and turn it in.

There is nothing that has officially released that suggest we need to do that. I have a feeling they won't want us to turn in every single document we have. This will only slow up the process because they will have to examine and verify every single paper. Gathering extra documentation won't hurt, but I can't imagine them wanting us to send in boxes of documents.

but its the only thing i got as far as medical records. most have my address and date printed and stuff.

I don't think you will be required to show medical records, but it is an option to use as proof. Anyway that should count as proof.

i am not about to call my dermatologist/physician/opthamologist for all these paperworks. they gave me a copy of blood work and a copy of my shots from way back in 96, and a couple stuff from nursing school. some of these are wayyyyyyyyyy overkill.
