Do we need to provide proof of presence for each and every month for last 5 yrs or couple evidence for each yr should be fine?.

No, you don't need proof for every month. Documentation that shows you were in the country before June 15, 2007 will suffice.
Things like vaccination records, school transcripts, tax returns, etc.

Not necessarily since we need to prove that we haven't left and then came back. You need to show that you have been consistently in the U.S for the last five years.

I phoned the hotline and they said three or four pieces of information spread out within a year is sufficient. If you have transcripts that show you were here from 2007 - 2012, you are fine.

It's older people that have graduated (I graduated in 200 that would need things other than transcripts in order to prove that I was here and never left.

Here's what I did that you could probably do:

= Identification documents
- Passports (Old and New)
- I-94(not really required it seems?)
- High school IDs
- College IDs
- Insurance

= Middle School records

= High School records (year)
- Transcript
- Diploma
- Exit exam certificate

=College records
- Transcript and what not

= Records from the last 5 years
- Bank receipts/records (2007, 2008, 2009, 2010, 2011)
- School records (No need for doubles)*only if you just graduated or have/is attending any educational foundation*
- Dental records
- Medical records
- Billings under your name

= The other stuff I included in my "evidence" pile
- Selective service card

Other stuff I'm thinking of including:
- Movie stubs
- Pictures
- Bus passes(haha don't ask)
- Veterinarian records

That's a GREAT list to go by!

I think transcripts are golden over diplomas since they can prove that you were here definitively during those years.

Your high school transcript would show that you graduated. If you entered college, then it also SHOULD demonstrate that you graduated from high school since you need to in order to get admittance.

The other stuff I wouldn't include though. That might be too much paperwork, you know? It might take a lot of effort to sift through UNLESS you don't have anything else.

Hmm.. this may seem like a stupid question, but how exactly do you request your records from your school? I switched states last year and in order for me to get records of my middle school and first two years of high school education, do I simply call the school's main office and ask them to email it to me?

For me, I called them up and asked and they then told me to come by to show proof of identification, then they wen't to the records room, printed my entire transcript. Easy Peasy.

Middle School, I'm not so sure but most likely the same process. Even if you attended a few years, they'll have records of you attending.
