I live at ____________________. I have known Maria Antonia Ramos for five years, since she was nineteen years old. I am not related to Maria Antonia Ramos.
I first met Maria Antonia Ramos in 2007 when she lived next door to me. Yes. Absolutely. The notary public would need to verify the identity of the affidavit signer.

When I went to the notary she told me that the last line is not necessary anymore and she included a paper on the back of it called "jurat"? and took fingerprints of my bf as well on that paper.

I have a similar situation in that I want to write a proof of residency for my landlord to sign stating that I've lived here and have been present on June 15th. The problem is I can't really convince him to walk all the way to the consulate with me to sign it in front of a notary.

Should I just write an affidavit similar to this one and get a friend/family member to sign? I looked at the checklist pdf of documents to assemble and it says no affidavits accepted.

From what I've read on this website you can get it notarized at the nearest bank. You don't need to go to your consulate

You need the affiant's birthdate, profession, and an explicit statement that the affiant is a United States Citizen.

Also, the affidavit needs to end with

"Further, the affiant saith not."

and

I SWEAR OR AFFIRM THAT THE ABOVE AND FOREGOING REPRESENTATIONS ARE
TRUE AND CORRECT TO THE BEST OF MY INFORMATION, KNOWLEDGE AND BELIEF." That is the 'canon' oath for an affidavit, I've been told.

Do they really need to be USC? Where did you read that?

Sorry, not USC (mines were, so that was my mistake). But their immigration status needs to be stated (Green card, EAD, Temp, etc.), and my lawyer told me affidavits from USC are "better".
