other names used? includes misspelled names?

hey everyone. just a quick questions, on the form where it says "other names used" like on immunization record the last name has 2 extra last letters. it sounds the same though. because in "other names used" i'm putting down any names i've ever used including any misspelled names caused by any agency!

Other names used means other names you have used, but have legally changed. As carnivore stated, make a note that the name was mispelled.

For a long time I went by my middle name "Alejandro," so my dental records have me listed as

Alejandro Contreras, instead of my full name xxxx Alejandro Contreras-xxxx

Just make a note of the error, and as long as everything else is correct, and you put on your forms the name given in your foreign ID (passport, matricula, birth certificate, etc) you will be fine.

what if my I-94 miss spelled my name but my passport has my correct name what should I do then?

Just mention that it was incorrectly spelled. Other names used means names you previously had, but have legally changed.

If you used to be Jon Doe, but have since changed your name to Jon Deer, put "Jon Doe" as previously used name.

What if some of your medical records show your short name. e.g. Maximilian <-- ful name, Max <-- nickname. Should i put that down as other names used?
