I am mailing it first thing tomorrow morning (via USPS Priority Flat Rate)

● Form G-1145, E-Notifcation of Application/Petition Acceptance
● Form I-821D, Request for Deferred Action
● Form I-765, Request for Employment Authorization
● Form I-765 Worksheet
● $465 Money Order for Form I-765 & biometric fees
● 2 passport style photographs
● Evidence packet to supplement form I-821D


Evidence packet to supplement form I-821D

Exhibit 1: Proof of identity and age [under 31 as of June 15, 2012]
- Copy of current passport
- Copy of Mexican Consular I.D. (front and back)


Exhibit 2: Proof of entry before age of 16
- Copy of expired passport bearing stamp of arrival (08/09/1990)
- Copy of I-444
- Copy of elementary school diploma (dated 06/25/2002)
- Official Chicago Public School records


Exhibit 3: Proof of continuous presence in the United States from 6/15/2007 to present date
- Official college transcript (Spring 2007 – Spring 2010)
- Copy of college associate’s degree (05/15/2010)
- Copy of credit report (March 2006 – June 2012 on page 4 of 8 )
- Copy of Capital One Credit Card 2011 annual statement
- Copy of Citibank Credit Card 2011 annual statment
- Copy of Discover Credit Card 2011 & 2012 annual statements

I don't think a cover letter is necessary uscis never mentioned it.

It's not necessary, but it's a good idea.

Use Express mail, FedEx, or UPS if you can afford it.
I have not seen anyone receiving Acceptance Notice who sent their applications to P.O. Box address using Priority mail.
Just my 2 cents.

Agreed. Express Mail will cost you a bit more (it's about $20), but it's really nice to see your package get there the next day and it seems like those sent using Express Mail/Courier have been processed first.
