Hello, how are you doing, I am posting this to see if anyone would be able to let me know if I am missing anything before mailing in my packet, and or how I should organize it, etc.

I entered the US in July 2000 with a tourist visa however my mother lost that and my 'I-94 Registro de salida' which my cousin who is also mailing in has. I hope what I have is enough proof, I can't prove that I entered in july as my earliest record is from august 3rd 2000, my vaccines, and enrolled in school in sept 18, 2000.

I currently have my:
High school transcript, I graduated june 2011 and that lists two classes in middle school that gave high school credit, one in 2005 6th grade, and one in 2007 8th grade, and my full high school schedule from the begining of 2008 all the way through my last year in 2011.

My immunization record, scanned both sides, which has the dates aug o3 2000 for my vaccines, my name, alergies, etc.

My matricula consular card which was issued 13 jan, 2012

My mexican passport which was issued 13 jan, 2012

My ITIN letter, with my ITIN number dated 11/16/2004

My high school diploma

My birth certificate (how would i go about translating this and is it required)

My bank statement from may 26-june 26 showing purchases before, on, and after june 15.

Also on the forms where it asks for previous addresses I don't even know all the places I lived nor have any proof, I can list the ones I lived in since about 2002 anything before that I can't recall.

Most public notary do translations. And yes it is necessary to translate your birth certificate.

birth certificate is not even necessary, your passport and matricula are good enough for proof of ID.

What I don't see listed is your time from after graduation to present.

Remember you need to prove presence at the time you submitted your application.

And don't worry about the addresses.

hmmm..if you have any report cards, that would help (especially for year 2007). also you have to submit two passport pictures, some people didn't and got approved but they are asking for it so submit it.

Hmmm, since graduation the only thing i could possibly use is my phone bill but it is under my moms name, i also have a couple of bills under my name since around march 2012, moms tax returns for 2011-2012 with me as a dependent? My diploma was issued june 20 2011 and my matricula and passport on jan 13 2012 so from that range i dont know what to show, i opened a bank account march 2012
