Is it okay to write your Alias as your middle name on your social security application?
Let's say my name is Ben Dover on my passport, but since I have ugly name I decide to use Ben (first) Chris (middle/alias) Dover(last). Is that okay? do I need to have some sort of a document to prove that I've been using name Chris.

and sorry guys i know this has been asked many times but since i'm posting a thread i want to ask again.
under category 5. do i check off legal alien allowed to work?

Thanks in advance!

The Social Security card will have whatever name your EAD has. Mine came with both my last names (mom's and dad's) and I asked the SSN lady if I could only have my father's she said that since the EAD card has both it will have both on the SSN card.

And probably also my DL, so whatever name you use on your EAD app it will be on the SSN.
