I also only showed my EAD, birth certificate and cosular card for my address. Though the guy helping me asked no questions except to see my EAD. I didn't receive any proof that I had applied.
I just called SSA and got disconnected.
I'm going to try calling again and asking if I could go back and get a print out.
The lady on the phone asked if I was calling for a replacement. I said no, I had applied I had no proof I had applied could I go back and see what is going on? I'm guessing she was going to respond but I got disconnected.

All you need to bring to get your social security card is your EAD and a identification document be it your birth certificate or your passport (they prefer a birth certificate). If you don't have either, if you're Mexican, you may show your matricula consular. There are other documents you can bring though.
