Sending my DACA. Is there anything wrong in my listing?

Hello! I seem to be a late sender here. I suffer from depression and the semester really took a toll on me. However, I'm almost done but I just need some critique on how I am finishing my app. I welcome them.
  1. (2) Passport Pictures, Recent
  2. Check of $465, (What to put in Title?) Mailing in Arizona because I am in California; Correct?
  3. Cover Letter
  4. G-1145
  5. I-821
  6. I-765
  7. I-765WS
  8. Table of Contents for Proof of Eligibility
  9. Supplied Documents, Tabbed from A to E
  • Exhibit A: Proof of Identity and Age
  • Exhibit B: Proof of Entry before Age 16
  • Exhibit C: Proof of continuous Presence from 2007 to Present
  • Exhibit D: Proof of Presence in June 14, 2012
  • Exhibit E: Additional Evidence (Receipts and Bank Summaries and Volunteer IDs)

All of the above, fastened. Some papers are stapled because they belong together (which is okay, right?) Tell me that I'm being very specific but I am serious as anyone who have done it and I'm doing it alone. Do I have something wrong?

* don't worry. your list seems fine.
* on the check put us department of homeland security
* 821d not 821
* make sure you provide proof from june 2007 to present
*make sure you keep copy of all the forms 821d 765 765ws.
even if you miss somthing they will ask for it, so be happy and just send it

Your list looks good and to the point. Please make sure that any additional documents that you send fall into one of those categories listed and if they do, make sure that you label them accordingly. Try to avoid any redundancy. From what I see, you are definitely on the right track.

The only error I noticed which may just be a typo is ''Exhibit D: Proof of Presence on June 15, 2012'' Please make sure that you write 15 NOT 14. Apart from that everything looks good. All the best!

Jessebrenele: How did you know to provide a cover letter? The USCIS website doesn't mention anything about having to provide one.
