Ready to send in files

Probably has been asked, but here goes.
Im trying to send my DACA today, little late..

1. Under the age of 31 as of June 15th 2012.
*No Birthcirtificate, but I have passport not expired.

2. Came to USA before reaching my 16th Birthday.
*I have Passport from Childhood that I entered United States under Visa at 1999 July 20th.
*Also I have visa papers

3. Have continuoisly resided in the US since June 15th 07.
*I have transcript from 05-08
*Membership of 24/7
*School ID card copys of 02-03, 04-05, 08-09, and a college ID card of 2010 (college ID card year sticker is missing)
(Can I just find any paper with my name in it thats yearly? like 07, 08, 09,...2012

4. Physically present in US on june 15th of 07 and at the time of making the request for deferred action.
*Bank statements of all of June.

5. Entered without inspection before june 15th 2012 or your lawful immigration status expired of as 06.15.2012
(I lost the Visa paper that tells me when the Visa expires, I do however have a Visa paper previous from that one that tells me it expires at 2007. I lost my Visa from 2007-2010)


6. Currently in school or have graduated or GED.
*Obtained GED in Febuary 2013. Will be sending copyed GED paper.

What do you guys think? am I missing anything? Also how much money do I have to send with this also in check? $465?

Just make sure the documents have your name, a date, and a general location (Ex. your address, facebook post/check ins, paypal/amazon receipts, bank statuses) and they should cover the years 2007-present. They can be yearly or monthly, but that is up to you and how you want to format everything. Some people did separators and an index so that is also an option. Looking good, good luck!

You write it for the US Department of Homeland Security, and yes you can send a money order. Tip: Make copies of it, of everything you send including the application itself!

Basically any evidence that has your information on it and most importantly the your name and date. Also and this might be helpful for you. On the DACA application first page I put down my full name as it appears on my birth certificate and passport (First,Middle,Last and Maternal Last Name). Then on another section it asked if I used another name. On that section I wrote down only my first and last and explained that in some evidence that's how my name appeared.

Also, if you lost something like let's say for example the i94 let them know. I lost mine and in the very back I wrote that I had lost it as well as in the cover letter I included.

Is your expired passport stamped? If it is make copies of where it's stamped!

Also for the 4th part and someone please correct me if I'm wrong but don't you have to send evidence of you being here after june as well since you are barely sending in the application in February?
