hi guys my question is from form I-765
on question 11 i answer yes so i have to complete the questions
where is says "Results (granted or denied- attach all documentation)"

WHAT DOCUMENTATION ARE THEY TALKING ABOUT?

I personally attached a copy of my employment authorization notice of action. Others didn't and just included a copy of their EAD.

Approval - 10/2012.
Renewal #1 - 10/2014.
Renewal #2 - 9/2016.
Renewal #3 - Pending.

A copy of your EAD and I-797 should be fine. Like MitchAPalooza mentioned, some on here have included only one.
