Okay guys. So here what my packet consist of as of right now

I-821D form
I-765 form
I-765WS form
G-1145 form
2 passport photos (about to go get this in a minute)
$465 Check or Money Order made out to 'U.S. Department of Homeland Security'
1 copy of your EAD card the front and back
Copy of the letter of I-821D and I-765 from my approval

here are my final questions:

(1) Do i have to send the two passport photos? I have read yes' and nos' ?

(2) On the I-765 Q#11 ask if we have applied, I clicked yes, but the follow up question
"Which USCIS OFFICE"
what exactly do i put?

On my envelopes i have one from the following

"USCIS CALIFORNIA SERVICE CENTER
P.O BOX 30111 Laguna Niguel, Ca 92607"

and

"Department of Homeland Security
P.O BOX 3011-30115
Laguna Niguel, Ca 92607'
which one should i put on the form?

(3) On I-821D #15 i selected Hispanic or Latino
which means i should leave #16 blank correct?

(4) Do we have to fill out any part of the form that say "for initial request only" some only say For renewal only

(5) This time on the forms, should i include the SSN they gave me after applying?

Race is associated with biology, whereas ethnicity is associated with culture.

White refers to people having origins in any of the original peoples of Europe, the Middle East, or North Africa" who reside in the United States.

Black refers to having origins in any of the Black racial groups of Africa.

American Indian refers to a person having origins in any of the original peoples of North and South America (including Central America) and who maintain tribal affiliation or community attachment.

4) Only fill out the sections labeled for renewals only.

3. You can leave #16 blank if you selected Latino. It won't be a deal breaker if you leave #15 and/or #16 blank.

I'm also trying to renew it, but what did you put for #15 on i765?

#16 is asking your race are white, black, native american, or asian. Do not leave anything blank that you can answer and risk getting it return.

Ohh.... so i answered #15 hispanic/latino... and #16 white.... will that be a problem? I already sent my renewal yesterday...

Okay, I went back to change it. My application looks really empty now. I have a lot blanks. I only filled out Renewal only

Even if you leave the ethnicity/race questions blank, you won't risk anything. These are just for statistical purposes.
