Who would you send the Manila envelope of all the stuff to? For the initial one It was sent to one center and then was moved to another. Would I still send it to the one designated for Florida, or to the one where the initial one was approved?
Also, even though it's not required, should I include photocopies of the front and back of the employee authorization card and notice of action?
