name used (i765 form)

this in behalf of my friend who is about to submit his packet. he has 3 names plus his middle and last name.
for example:
full legal name : Juan Paulo Max Jane Doe.
JUAN PAULO MAX is his given names.
JANE is his Middle Name
DOE is family name.

on form I-821D he put other name as (14.a) DOE (14.b) Juan (14.c) Jane,
on Part 8 (additional info) (1.a) DOE (1.b.) Juan Paulo (1.c.) Jane

on form I-765 (1. Full name) Doe Juan Paulo Max Jane (2. other name used) Doe Juan Jane

his concern now is to where to put the Doe Juan Paulo Jane , because unlike with form i-821D it has extra sheet for additional info, form i-765 has no extra sheet for additional information, as this name also appeared on some of his documents.

please help.. we appreciate your input.

If the name field is too short for his full name I'd print the form and put the name in with a pen.

i will print another form i765 and put the other name on it?

That's what I'd do. Leave the name for the field blank, print the form, and put the name in there with a pen.
