Hi, could anyone help me file my return regarding the health care.

I have a health insurance sponsored by my employer however I did not have an insurance for the beginning of the year.

Jan- April: No Health Insurance
May - December: Employee Sponsored Health Insurance

How should I fill out the necessary forms? Thank you in advance.

You can file IRS form 8965, Health Care Exemption. You qualify for an exemption under DACA. If you're using tax software, it'll fill the form out for you.

Do you think it's better to claim exempted for the first 4 months using the 8965 Plan C then declare the remaining 8 months that i have health insurance through company sponsored OR just file that im exempt and dont bother declaring that i have company sponsored health plan? Is there any tax implications?

Well the exemption is good for the entire year. Your employer will still send your insurance information over to the IRS. So you might as well put the entire year and then next year, you won't need to file the exemption

So for 2015 tax filing just claim that I'm exempted for the healthcare?
And if I stayed in my employer with the sponsored health plan for the entire 2016 then just declare this for 2016 rather than claiming the exemption?

It doesnt matter if you have or dont have health insurance, you still put youre exempt.

For the months you did not have health insurance you are EXEMPT because you are a noncitizen. Also because you are not eligible for insurance through the marketplace. Make sure they don't penalize you for not having health insurance!
