The instruction of form i765 say I94 is a required document, but since it is not new (I sent it in initial request), is it required?

You don't need to submit anything unless it's new.

If you travel on AP - yeah send in the new I-94. Otherwise no.

If you already sent it, you don't have to send it again. For my second renewal I did include just for the heck of it. For my current renewal I did not.

I strongly suggest you still include proof of it, back in 2014 I used AP and came back to submit my application, I included it in 1st renewal and as the people here are recommending, did NOT include this in my 2nd renewal, a month before I expired, I received a NOID asking to clarify that departure.
