Question about AIW Worksheet

I am currently filling out my worksheet for my appointment tomorrow but I don't know where to put my moms last name on the line? Does it go on the middle name area or the last name area?

or does it go together with my dads last name on the last name area?

Hope i'm not confusing yall.

As with any form, your mom's last name would be on the last name area.

If your name is Alfredo Carlos LOPEZ ROMERO, then:

First name: Alfredo
Middle name: Carlos
Last name: Lopez Romero

If you don't have middle name, leave blank.

Nope, that's only if you had a different name in the past. For instance, if you're married now and you took your spouse's last name. They'll want to know what your last name was before that.

If your name is Alfredo Carlos LOPEZ ROMERO, but you go by Alfredo LOPEZ then you don't need to put anything in the alias section.
