You can go with the name that you prefer to use, but they may just go with the passport name since that is considered the government name.

These ASC's man... On the biometrics letter it allows for any state issued or government issued ID. It does not restrict it to passport.

Don't worry about the old application addresses. If USCIS cared about the discrepancies, they would have RFE'd the inconsistencies. You were approved so I would renew and not worry.
