I was careless and assumed that my g-1145 was in my package before I sent it out. It turns out it wasn't. I forgot to include it. T I'm just over thinking things, since that form isn't a requirement.

Under I-765 it says Which USCIS Office? what do you put there ?

Also under that it says Results (Granted or Denied - attach all documentation
i believe that pertains to if you had a work permit before. if you have never filled form i-765 before, then say no and leave it blank

You can write it out. If you are doing USPS express, you don't even have to write anything on the envelope. When you get to the post office they will give you this form to fill out which they stick on the envelope which has both your return address and where it is being sent.

Question: do you have to get your own criminal record (state) and include it to your docs? Some sites that give DACA filing advice "strongly suggest" you to do it. Has any of you sent theirs?

If you have a clean record, I see no reason to do this. DHS will do a background check on you anyway.

Just go in and ask. The worst they can do is tell you to come back for your scheduled appointment. They don't take phone calls.

You can also call USCIS at 1.800.375.5283 and ask directly.
