I was careless and assumed that my g-1145 was in my package before I sent it out. It turns out it wasn't. I forgot to include it. T I'm just over thinking things,since that form isn't a requirement.

Yes, unless it was a life or death situation I wouldn't reschedule. Seems like they give you plenty of time to plan around it ei asking time off from work etc.
