Help with filling out forms for renewal!

1. After that segment, questions 12 through 17 start. Do I have to fill them out or leave them blank?

2. Also, do I have to put the USCIS number on my EAD card where it asks for an Alien Registration Number?

3. For the USCIS office, do I put "Vermont Service Center" ? (that's what it shows on my approval notice letter)

4. According to this thread and checklist (http://dreamact.info/forum/showthread.php?t=68487), I need a copy of my I-94? Is that even necessary? And also passport photos? What do I really need to send?

4. you only need copy of your EAD front and back... and just in case copies of your approval letter(i-821d and i765) and 2 passport photos
