letter stating my presence on my behalf

I volunteer under a doctor and it's the only thing I do these days. I need want her to write a letter stating that I was here on June 15th, but ofc she refuses to do anything herself. I have to write the letter and then she'll sign it. I just want advice on how to make it look official enough and have all the pertinent details.

The letter must have her clinic's full information and ask her to stamp it too.

What if she doesn't have a clinic, she is employed by various nursing homes. Do you think that i should get it from the nursing home instead?

Sure, do you have her business card too? Put those information on the letter as well.

thanks for the advice, i just called the nursing home. i'm waiting to hear back from them. i'll try to get her business card too, if things work out.
