My friend had to get one a few months ago and she said they asked her some questions about her medical history, vaccines, checked blood pressure and her lungs and took some blood. She said she didn't have to get naked. It was non invasive. I was curious about the exam so I called the doctor that's in my area who does tests and the receptionist pretty much said the same thing as my friend and also said that the doctor doesn't require you to remove your clothes unless you tell him something that would make him wanna have a look. For example, if I tell him I have a lump in my breast or something, then he'll want to check that out.

Was your friends physical for immigration purposes though?

Jadee is correct you don't have to remove your clothes as i stated earlier , AOS means adjustment of status
