I just applied online. Easy process, pretty much the same questions. At the end, they show you the forms all filled out by machine. Also, you can now pay via credit card.

1 - First file for i825d.
2- Then it'll prompt you to the i-765. They ask you for the 2x2 photo of yourself when doing the latter, also you have to scan your filled out i-765WS form. Highly suggest you leave about an hour to do this and have a scanner readily available for the supporting documentation.
3 - Payment page gave me an error initially, but luckily it saved all my responses, so all I had to do was re-load the page.

10/10 - would online re-file again.

Did you scan in your 2x2 pictures?
