What to put for "In Care of Name (If Applicable)" I live with my parents, do I put my mom or dads name in there?

No don't be concerned. they are all going to nebraska. and that's a good thing, it means your case is moving along smoothly.

I need some help.
I got the same notice yesterday. After looking at a few people's status... either you get the "Routed to USCIS Nebraska" or " we received I821D Consideration of Deferred Action"

I have already been through removal proceedings and I'm thinking maybe people with A number are getting forwarded to Nebraska... What is your status?

That is incorrect. ALL applicants are required to submit 2 identical 2” x 2” passport photos in addition to other specified documents such as the check or money order, etc.

In the I-765 instruction under "Required Documentation" , section 2, sub-section B, it states the following: " If you are filing under the (C)(33) category, additional documentation beyond what you submit under question 3 on page 3 of the Filing Instructions for Form I-821D, Consideration of Deferred Action for Childhood Arrivals, is not required." Now when you go to that section on the I-821D instructions form, it says "What documents do you need to prove your identity?: Passport, Birth certificate, National document from your country, ETC.... It doesn't say anything about the passport photos. I know that in the I-765 instructions after section B in section C it says you do but thats after it has said no further information is needed for (C)(33) applicants. Correct me if I'm wrong.

Put "U.S department of homeland security" just like that for the pay to order of part.

Don't really need an address. Just use the one you are sending it too. Just make sure it is made out to them!
