For those who have received any mail (797), was it addressed to you with both last names (given that your birth certificate has your mothers and fathers last name)?

Has anyone else who got their packet delivered to Chicago on 08/20 not heard anything back yet either?? I'm worried

And by delivered i mean only the mail carrier's notification.

I just received my college transcript and on the SSN number there are 5 asterisks and 4 actual numbers. i NEVER submitted any sort of false social security number in fact I had to sign an affidavit...

We dont have to fill out parts 5 and 6 of I-821D(preparer's information) right ( if we are filling it out ourselves)??
