Not sure if this situation has presented itself before but might as well ask. A little background first.

I had already gathered all my evidence and documents and I was ready to put the packet together on my own, however there was a free DACA clinic at a university here and I though, I'll go and see if they can confirm all my documents are in order. I get there and there are way more people than I expected, but I decide to stick around. They tell us all the stuff that I had already read, basically all the stuff from the USCIS guidelines and the form instructions. I end up going through the whole process they had set up to prepare my stuff so that in a final review with one of the immigration attorneys that had volunteered their service for this event, they could just assume everything was set up and they could just focus on our case. Basically, going through my documents a couple of times, verifying all proof needed is there, and what not. Stuff I had already taken care of. This stuff was being done by folks that had just graduated law school and I'm assuming where getting some real life practice, this came with a couple of problems as well, here's how. I go to the second girl who was supposed to check my stuff again and actually start filling up the application with me. I pulled out my laptop and showed her I had already done that too. We started reviewing it, and in question 9 part 1 of form 821(residence) her and a lot of the other graduates were putting their country of origin instead of united states. I had US already there, and she said that was wrong, and I asked her if she was sure, because place of residence seems to me clearly to mean the country where I reside. She started babbling on how immigration might be referring to the country of legal residence, but I told here that's why the country of birth was there. We skipped that question and 2 minutes after, a bunch of people were coming back to the people that had filled their application. Turns out the attorneys noticed this and said they were putting the wrong thing. I think they were doing this just so these practitioners could put their name in the section where it asks if an attorney helped you filled this form.

Anyway. I went through this check, just because I wanted the actual attorney to go through my stuff with me. They put all my evidence in a folder, which went to the bottom of a big stack. This was the stack in front of the room where the attorneys were taking people one by one, they said that was the final review, and when I had talked to the attorney they were going to make copies of all my stuff and put the packet together, and I just had to provide the money order. I said ok, I had made it that far, so I sat there. I first got there at 10 in the morning, it was 5:30pm and I still had about 15-20 more folders before mine, and each folder took about 15 to 20 minutes. I was tired, not having had lunch, some of the attorneys were leaving because it was past 5, and I had another appointment for a little job at 6:30 so I decide I wasn't going to wait. I pretty much knew I had my stuff together so I asked for my folder and they started questioning, on why, and that I would have to make an appointment for a month later if I wanted another chance to talk to the attorneys, and that they had to take the application they had printed because they had helped filled it. I told them I didn't mind and I just wanted to take my stuff and leave.

They finally gave me my folder after digging through the big stack they had. I put it in my bookbag and I headed to the bus stop. I was in the bus and I decided to check my folder to see if everything was still in order, and surprise, my passport is missing. They probably dropped it when they were looking for my folder or something. I was very pissed, but I wasn't gonna go back because they had already started telling people they could come next week because they were going to finish in half hour anyway. I called the office of the university department that had organized the even to see if they had my passport and no luck. Great. I head to the consulate and get everything set up for another one, but they said it'll come in 2 to 3 months.

I know a couple of people that work at the consulate and they helped me get my passport the first time, so when I told them what happened they said they'll try to put an urgent request for my passport but it might still take 1 to 2 months. The other thing they did is give me a provisional passport.

And that's my question. Does anyone know if USCIS would accept a provisional passport?
I'm asking because, this provisional is all in spanish and it's basically a sheet of paper with all my info, passport number, DOB and what not, as well as some official stamps from my country. But it still doesn't really look like a passport. I have my birth certificate and it's translated, but I also wanted to send a copy of my passport with my application. Should I wait it out for my passport or what? Any suggestions?

On the 8.a section, do they want only the city of birth or city and state?
And I need to sign the "Signature of Requestor" by hand correct?

Part 5, and 6 are only need if an attorney is handling the application, right?

Do you guys think I should get my I-94 for the purpose of applying? or like the Form says leave it blank if you don't have it and Check mark no on the I-94 question.

Depends on your situation. You certainly don't need it but having it will give you a big advantage of proving when you entered the country.

Nice story, but you really only needed the last paragraph lol

I'm pretty sure they will take provisional passports, but all you really need is a birth certificate (translated and acknowledged by someone else). Make sure you also translate your provisional passport as well.
