if you have a valid passport, you don't have to send a translated birth certificate.
i forgot what q#15 is, but i believe it's the one that asks for current status? if so, most people have included a variation of "unlawful status: DACA"

you have nothing to lose, unless you have a criminal record. If you get denied, they won't deport you (again, unless you have a criminal record/are a terrorist)
so use both. tax applications, affidavits, etc. for the affadavits, make sure it's notarized and your address is included. you need to read the instructions, it gives more information about what they expect.

I also received one that typed like that. I also received an 11 page shcool history from K-12 grade that I requested from the school board. All for $0.
