Mail the forms so that the arrive no earlier than the first date of implementation.
Anything that doesn't apply write N/A
Check your passport US Visa to get the Visa type (visitor, student, etc.)...

I got the "you card is being sent for production" on Monday last week. On Thursday I received "your card has been mailed", and on Saturday I received the card.

They are probably processing end of August applications at the moment. Mine was received Aug 26 and was approved on Monday.

Called today USCIS today. Apparently they are working on applications received August 16 as of their last update (they update every month, last month it was July 26 I think).

My RFE was a plain envelope, white sheet requesting some evidence and no envelope to send it back. I sent my evidence on a plain white envelope.
